Objective: Weight 18.2 kg (75th percentile), height 105 cm (80th percentile), BMI 16.5 (50th percentile). Alert, active, and cooperative during examination. Cardiovascular, respiratory, and abdominal examinations normal. Developmental assessment: Speaks in complete sentences, draws circles and crosses, hops on one foot, counts to 10. Vision and hearing screening passed.
Assessment and Plan:
